If you are pregnant and currently in between 10 to 14 weeks of gestation, it’s time to submit yourself to one of the pregnancy tests which is known as the nuchal translucency scan (NT scan).  It’s to measure the nuchal area which is a normal skinfold situated at the back of your baby’s neck. This test can be conducted when you are about to end your first trimester or during the start of your second trimester.To put it simply, your doctor will measure the thickness of clear space located at the back of the neck of your foetus. This is to determine whether they are at risk of developing defects related to chromosomal abnormalities such as Down syndrome, Patau syndrome and Edward’s syndrome. For these reasons, most women would feel anxious. However, knowing your NT scan result can give you a peace of your mind or prepare you for what’s to come. 

Why might I have a nuchal scan?

All pregnant women are offered a nuchal translucency (NT) scan, to assess whether or not their babies are likely to have Down's syndrome. It is part of the detailed scan you will get in your first trimester.

Every woman has a chance of giving birth to a baby with Down's syndrome. The chance increases as you get older. A screening test tries to get a clearer estimation of whether your baby may have Down's syndrome. It can't tell you for sure.

Diagnostic tests, such as chorionic villus sampling (CVS) and amniocentesis, can say for sure whether or not your baby has Down's syndrome.

However, diagnostic tests carry a small risk of miscarriage. Because of this, your doctor will want you to have a screening test before she recommends a diagnostic test.

The screening test combines the NT scan result, with a blood test and other factors, such as your age. The test is designed to give you as much information as possible, so you can decide whether or not to go ahead with a diagnostic test.

How is a nuchal scan performed?

The scan is usually done through your tummy. A full bladder is not required. The ultrasound doctor will put some gel on your tummy and move a handheld device (transducer) over your skin. It shouldn't hurt, though you may feel some pressure as the doctor presses the transducer into your skin to get a good view. Sometimes, it's hard to get a good view through your tummy, in which case your doctor will recommend a transvaginal scan (TVS). This is particularly likely if your womb (uterus) tilts backwards, or if you're overweight. You will need to empty your bladder for the transvaginal ultrasound. Rest assured, a TVS is safe for you and your baby, and shouldn't be too uncomfortable either.

 

The doctor will start by measuring your baby from the top of his head to the bottom of the spine. This will help her to date your pregnancy accurately. She will then measure the width of the nuchal fluid at the back of your baby's neck. The skin will appear as a white line, and the fluid under the skin will look black.

You'll usually be able to see your baby's head, spine, limbs, hands and feet on the screen. Your ultrasound doctor will be able to rule out some major abnormalities, such as problems with your baby's abdominal wall, stomach, urinary bladder and skull, at this scan.

But a more detailed anomaly scan (ultrasound level II), also known as a TIFFA scan, will be prescribed between 18 and 20 weeks of pregnancy. By this time, the doctor can see your baby's features better, so the anomaly scan gives a better idea of how your baby is developing.

A multiple pregnancy occurs when one egg (ovum) splits before implanting or when separate eggs are each fertilized by a different sperm. Identical twins or triplets occur with the fertilization of a single egg that later divides into two or three identical embryos. Identical twins or triplets have the same genetic identity, are always the same sex, and look almost exactly the same. Fraternal multiples develop from separate eggs that are each fertilized by a different sperm. Fraternal twins might or might not be of the same sex and might not necessarily resemble each other any more than two siblings from the same parents might.
